Once a bone has been formed, it is continuously being remodeled throughout your life.
This process involves the action of osteoblasts and osteoclasts, two hormones (calcitonin & parathyroid hormone) which in turn affect blood calcium homeostasis.

The rate of remodeling varies from distal femur being replaced every four months to the diaphysis which may not be fully replaced during one's lifetime.

Bone remodeling/ calcium homeostasis is controlled by the parathyroid hormone and calcitonin.

Other factors include the mineral calcium and vitamin D, C, and vitamin B12.
