One element that affects human health generally, and respiratory health in particular (lungs), is air pollution.

Many factors, such as a genetic predisposition to asthma, early exposure to allergens in childhood, excessive use of antibiotics, exposure to tobacco smoke (primary or second-hand), and/or exposure to air pollution (which can include both indoor and outdoor air pollution) can all be factors in the case of an individual with a lung disease. In many developing countries, such as India, indoor air pollution from smoky cooking fires can be a major health problem, especially for women and children who spend a lot of time indoors.

Therefore, it can be very challenging to conduct experiments to separate out each of these factors and how they directly affect a disorder.

For these kinds of studies, researchers occasionally used identical twins, particularly those who were raised in different environments.
